Project ECHO: A New Care Model to Benefit Clinicians and Underserved Patients

Description:

Join Dr Sanjeev Arora, Vice Chair of the Department of Medicine at the University of New Mexico, as he presents an overview of Project ECHO, an innovative and effective model for chronic care delivery to underserved patients. Developed and piloted in New Mexico, Project ECHO has built disease-based knowledge networks that link academic medical centers to community primary care providers in order to facilitate the delivery of timely and evidence-based care to underserved patients with complex health conditions. Recognized for its positive impact on patient health outcomes, healthcare costs and clinician satisfaction, Project ECHO represents a new model for chronic care delivery with primary care at its very heart.
