Biography

Holly Divine, PharmD, BCACP, CGP, CDE, FAPhA, is a Clinical Associate Professor in the Department of Pharmacy Practice and Science at the University of Kentucky (UK) College of Pharmacy. Her clinical practice site as an ambulatory care pharmacist is at Bluegrass Community Health Center, a patient-centered medical home, in Lexington, KY. Dr. Divine co-founded the PharmacistCARE Medication Therapy Management (MTM) Service where she served as the Program Coordinator for the American Diabetes Association-Accredited Diabetes Self-Management Education Program and practiced ambulatory care pharmacy for ten years. This award-winning patient care service is the recipient of the 2008 American Society of Health-System Pharmacists (ASHP) Best Practices Award and 2005 American Pharmacists Association (APhA) Foundation Pinnacle Award. She received the 2010 American Pharmacists Association Community Residency Excellence in Precepting Award and is a 2011 APhA Fellow. Practice interest areas include MTM, diabetes, geriatrics, cardiovascular disease, and self-care.
