Description

This program discusses differences between palliative and hospice care and reviews which is most appropriate in light of a patient's disease trajectory. We will discuss Advance Care planning and discriminate among the types of Advance Directive that are available. Best practices for initiating discussions regarding prognosis and the role of patient activation in serious illness will also be explored.
