Biography

Connie Lewis is a Heart Failure Nurse Practitioner at Vanderbilt University Medical Center in Nashville, Tennessee. She has over twenty years of outpatient heart failure clinic experience. Connie has developed numerous patient education tools, written protocols for the disease management program, as well as worked with information systems to develop an electronic identification of heart failure inpatients and readmission risk tool that has been studied and validated. In addition to· electronic HF Dashboard development, her other research interest is young adults with heart failure. She speaks nationally on a variety of heart failure topics. She served as the President of the American Association of Heart Failure Nurses. Her other professional affiliations include the Heart Failure Society of America, Preventive Cardiovascular Nurses Association, American Nurses Association, Tennessee Nurses Association, American Association of Nurse Practitioners, American Association of Critical Care Nurses, American College of Cardiology, Middle Tennessee Advanced Practice Association, and is a founder of the Middle Tennessee Heart Failure Journal Club. She has certifications as a Critical Care Registered Nurse and Certified Heart Failure Nurse. She is board certified as an Acute Care Nurse Practitioner and an Adult Nurse Practitioner. She is a Fellow Heart Failure Society of America.
