Overview

This presentation will provide updated guidelines for primary care providers to integrate chronic kidney disease care into routine practice. The presentation will be a case-based discussion. Through this presentation, we will review the prevalence and pathophysiology of chronic kidney disease. We will discuss updated screening recommendations and the workup for chronic kidney disease. We will also provide evidence-based interventions for management of chronic kidney disease.
