Overview

This session equips primary care clinicians with practical communication strategies and advanced care planning tools to support patients with serious illness in making informed decisions about their care. Participants will explore best practices for navigating difficult conversations in the primary care setting and reflect on the unique and vital role primary care clinicians play in walking alongside their patients at the end of life.

The session "Difficult Conversations, Meaningful Care: Navigating End-of-Life in Primary Care" (1.00 hour) is designed to satisfy state licensure requirements (NP Only) on Geriatrics in Louisiana and is also approved for Medical Ethics/Professional Responsibility (MD/DO) in TX.
